Questions and Question Types
Overview of the question types available when building forms and surveys
When Creating a Form, the question type options appear across the top. The type of question determines how it is created.

You can see how each question will look to users by previewing the form. This is done from the Surveys & Forms list Surveys and Forms List, and opens in a new browser tab.
Note: You can apply form logic Form Logics to certain types of questions. For example, if a user answers Yes to a question, they are brought to another question that does not appear to users who answered No.
Question types include:
- Free text
- Multi-choice
- File upload
- Date
- Group List
- Ranking
- Election
- Signature
- Quiz

- Binding Fields: An admin-only feature used in approval processes
Free Text
There are two types of free text questions: single-line (for short answers) and multi-line (for longer answers).
This question type accepts text, numbers, email, or URL. For multi-line answers, you can set a minimum and maximum word count.

Multi-choice
There are three multi-choice question types.
For multiple-choice questions with one possible answer, choose either single-selection or dropdown. If one or more answers are allowed, choose multiple-selection.

Click Add Answer to add each response.
You can align checkboxes vertically or horizontally, add a comment box, and randomize answers. For multiple-selection questions, you can also set a maximum number of answers.

File Upload
Use this question type to request a file upload, such as a photo or document.

Date
This question opens a calendar popup.

A date question would look like this in the form:

Group List
Users will be able to select from all groups that are included in the selected group types.

Note: This question type is often used by platform admins when collecting information about a group. Officers can select their group for which the form is being created. A Group List question can also be used in a workflow that uses a dynamic group Dynamic Workflow Teams: Dynamic Groups.
Ranking
This question enables users to rank options according to their preferences. You can choose to randomize answers.

Election
An election question enables users to vote for candidates, typically to fill various roles in a group. Note that an election question can have only one response, and will not work for ranked choice or slate voting.
Click Add candidate, and use a name search to list each candidate. Each candidate can also have a description, along with a video or other attachments.

You can also include a write-in option or a comment box below each answer, include a no-vote option, and randomize candidates. You can also limit the number of candidates a user can vote for.

Signature
Use this question type to collect a user’s signature.

The signature question looks like this in the form.

Quiz
A quiz is similar to a multiple-choice, single selection question. But for a quiz, the correct answer is provided, and users can earn points if they answer correctly.
Note: A quiz’s passing score is set in the Basic tab of the Form Settings.
Click Add Answer if you need to add responses, and indicate which answer is correct. You can assign a point value for correct answers, and randomize answers.

User Select
This question type enables users to select one or more other users. You can allow more than one answer.

Note: A User Select question can also be used in an approval process, when combined with a Dynamic User Dynamic Workflow Teams: Dynamic User.
Rubric
A rubric is typically used for officers or staff to score or assess form responses.
In this example, the user completes the first two questions of the form.

The next question is a rubric, defining the scoring criteria for the group member’s project. This question is for officers only.

The rubric question looks like this on the form:
